This study evaluated the concordance of surgical outcomes with the molecular test results from ThyGeNEXT + ThyraMIR/ThyraMIRv2 testing and the false negative rate in a cohort of cases with a Bethesda category III, IV or V diagnosis. This small retrospective review examined 19 patients who underwent preoperative ThyGeNEXT+ThyraMIR/ThyraMIRv2 testing followed by a thyroidectomy.

Post-surgical histopathology revealed a low false negative rate and high positive concordance of 89.5%.

The microRNA classifier was effective in detecting malignancy, even in cases with subthreshold mutations or without common genetic alterations. Key mutations identified included TERT c.-146C>T, NRAS Q61R, and BRAF V600E.

These findings further support the utility of ThyGeNEXT+ThyraMIR/ThyraMIRv2 in enhancing risk stratification and helping guide management of indeterminate thyroid nodules and thyroid cancer.

Patient management decisions are based on the independent medical judgment of the physician and molecular test results should be taken into consideration in conjunction with all relevant imaging, clinical findings, patient and family history, as well as patient preference.
