Utilizing Archival Cytology Slides to Overcome Molecular Testing Insufficiency Rates in Cytologically Indeterminate Thyroid Nodules: An Institutional Experience

ATA Annual Meeting, 2025
  • Molecular testing is an important adjunct in the management of cytologically indeterminate thyroid nodules (ITNs). However, test performance and specimen requirements vary among molecular platforms1
  • Our healthcare system recently transitioned from the Afirma Genomic Sequencing Classifier2 (GSC) to Interpace Diagnostics’ ThyGeNEXT® + ThyraMIR®v23 multiplatform testing due in part to high molecular insufficiency rates with Afirma® GSC
  • The switch to Interpace Diagnostics’ ThyGeNEXT + ThyraMIRv2 testing offered increased flexibility for specimen collection. Both Afirma GSC and ThyGeNEXT + ThyraMIRv2 accept liquid samples containing follicular cells collected during the fine-needle aspiration (FNA) procedure. However, ThyGeNEXT + ThyraMIRv2 also accepts cytology slides under 6 months old with >80-100 follicular cells
  • This study aimed to determine whether archival cytology slides from thyroid FNAs deemed insufficient (INS) for the Afirma GSC test could produce successful molecular results with ThyGeNEXT + ThyraMIRv2 testing

Patient management decisions are based on the independent medical judgment of the physician and molecular test results should be taken into consideration in conjunction with all relevant imaging, clinical findings, patient and family history, as well as patient preference.
